Our services include everything from giving personal care and homemaking to high-tech
treatments, therapies, and IV nutrition.
A-1 personnel have the training and experience to provide care to patients with
Alzheimer's and dementia, strokes, diabetes, heart conditions, and other chronic diseases.
Home Care
Who is eligible to receive PASSPORT Services?
- is 60 years of age or older
- is eligible for Medicaid**
- lives in a home or apartment
- needs hands-on help with daily activities (i.e. bathing, dressing, meals) to remain at home
- the person and her/his doctor agree to the care plan
- health and safety needs can be met by home care services.
- the entire package of services costs no more than 60% of the cost of Medicaid nursing home care.
Who is eligible to receive OPTIONS Services?
- is for those who are not eligible for the home support services through PASSPORT
- 60+ years of age.
- Residents of Cuyahoga County.
- Living in a private home or apartment
- Eligible for home support services based on an assessment of need for assistance with daily activities, such as bathing, dressing, food preparation.
- Managing expenses on a modest monthly income with limited financial reserves
Who Pays
- Medicare will pay if a patient is homebound and in need of skilled intermittent care.
- Medicaid will pay if a patient needs assistance with activities of daily living.
- PASSPORT and OPTIONS programs also cover home health aide services.
- Private insurance frequently covers home care. This can be explored on a individual
basis by home care staff.
- Client pays if not eligible for government programs.
